Here at FIREFLY our Centralized Intake processes referrals for the wide range of programs and services we offer throughout the Kenora and Rainy River Districts.
Services can be accessed for children and youth from birth until the age of 18, or up to the age of 21 if they are in school and living in Ontario.

If you complete the online referral form and you have not heard back from FIREFLY within 48 business hours (Monday-Friday 8:30 AM-4:30 PM), please call our Centralized Intake number: 1-833-696-5437.
If you are the parent or caregiver of a child or youth (up to age 18) and would like to access developmental services such as Physiotherapy, Speech-Language Pathology, Occupational Therapy, etc. please complete the General Referral form below as well as the appropriate service screener and e-mail the completed forms to [email protected].
If you are a youth (up to age 18) and would like to access Mental Health Services, complete the CYMH Self Referral Form here.

If you are a Primary Care Provider and would like to refer a child or youth for service(s), please click on the button below to fill out the OCEAN referral.

FIREFLY is one of Ontario’s SmartStart Hubs, a service available to families with children from birth to 18 years old. Children aged 19-21 who attend publicly funded schools can also access SmartStart Hub services.
