Coordinated Service Planning (CSP) is part of the Special Needs Strategy, an initiative of the Province of Ontario. CSP was created to support families with children/youth with multiple or complex special needs. CSP is about linking you to services and ensuring that all the professionals involved with your family are working together as a team towards your family’s goals and that you are kept informed of what is going on at all times.
Families can make a referral to CSP by calling Centralized Intake at 1-833-696-5437 or emailing [email protected].
