Coordinated Service Planning 

Coordinated Service Planning is part of the Special Needs Strategy, an initiative of the Province of Ontario. Coordinated Service Planning was created to support families with children/youth with multiple or complex special needs. Coordinated Service Planning is about linking you to services and ensuring that all the professionals involved with your family are working together as a team towards your family’s goals and that you are kept informed of what is going on at all times. 

Referrals/Self-Referrals

Families can make a referral to Coordinated Service Planning by calling Centralized Intake at 1-833-696-5437 or emailing [email protected]. 

You may benefit from Coordinated Service Planning if: 

  • you have a child or youth with complex special needs up to 21 years of age who is still attending 
    school; 
  • you are receiving or waiting for services from more than one service provider (including schools, 
    therapists, community supports, and medical professionals); 
  • you are finding it difficult to navigate and coordinate multiple services and; you want to ensure the best plan of care for your 
    child/youth 

Service Providers, including physicians can make a referral to CSP by submitting a General Referral Form found on our Get Services Now page.
